ON_CALL_RN after hours call service decreases the number of client ER visits, admissions and re-admissions. 

 

How this is accomplished:  The client calls ON_CALL_RN after hours regarding symptoms.  ON_CALL_RN, having access to client information and using physician driven Triage Software, reviews the client’s symptoms and follows established algorithms to determine if the client needs emergency care or if the condition warrants at home treatment with physician in office follow up.   ON_CALL_RN will recommend to the client Triage Software recommendations and follow up with the client to see if the recommendation has helped the condition or if they have any new symptoms.

 

Clients using ON_CALL_RN services will have priority physician appointment scheduling if their condition warrants a physician follow up.   This is to encourage clients to call early when symptoms develop rather than call the physician’s office after symptoms have been present for several days, when their symptoms may have worsened and then potentially be unable to see the physician for several more days while waiting for an open appointment which may lead to the need for the client to be admitted to the hospital. 

 

Client’s Physician will receive daily faxed report on all clients who received ON_CALL_RN services.

 

If it is determined that the client should seek emergency treatment, the ER will be called and notified that the client is in route and the emergency department will be faxed client information including, reason for emergency treatment,  a list of current physicians, a list of current medications and allergies and insurance information.

 

If physicians serviced chooses to provide ON_CALL_RN with after hour protocols these will be followed.    After hours protocol may include standing PRN orders, where they would like their clients sent in the event they need Urgent or Emergent Care, obtaining outpatient labs such as a urinalysis if the client has symptoms which reflect a possible UTI.   Labs will be reviewed and ON_CALL_RN will notify the physician of the results.   ON_CALL_RN will call the pharmacy of client choice for the physician with medications ordered by the physician.

 

Other services provided:  Client education on illness, medications, treatments or procedures which may be faxed or emailed to the client if they have access to a fax or computer.
